Why Care Coordination? Are you worried about who will look after you or your loved one when you are unable to? Do you need assistance but are afraid of losing control? Maybe you need someone to facilitate a need for care discussion with you or your family? Perhaps you want to stay at home and need someone to monitor the care you receive? Do you anticipate surgery and want someone available to advocate for your needs and help plan your post-operative care?
A Care Coordinator’s professional experience and unbiased approach helps to balance concerns among family members and weigh the factors involved in decision making. The goal of Care Coordination is to increase the control of the person served and save you precious time and money. The Care Coordination services of My New Friend are customized to meet your preferences and strive to maximize each person’s independence through creative and efficient use of services. We work for you to ensure that your resources are used wisely and the care you receive is appropriate and the highest quality. We provided education as needed on aspects on aging including in-home options, entitlements, advance directives, guardianship, estate planning, and if necessary alternative living arrangements. A care coordinator can assist with crisis management, complete an objective long-term care assessment, monitor care received, and if necessary arrange for successful out-of-home placement, all in partnership with you and your family. To accomplish the goals of our client the care coordinator monitors, listens, observes, educates, and advocates for your best interest.
If you choose to receive Care Coordination services and do not have a current assessment the care coordinator will complete one tailored to your needs and preferences. Person centered assessment is the cornerstone to quality care, and the services the Care Coordinator completes on your behalf are always mutually agreed upon and can be discontinued when no longer needed.
